KIN Launches the “Continuum of Care” Model for Comprehensive Stroke Rehabilitation
Protecting the First 6-Month “Golden Period” — A First in Thailand
KIN Rehabilitation & Homecare has created a seamless stroke-care system from the patient’s bedside to the front door at home, connecting advanced-technology partners along one integrated pathway.
Key Information | Key Facts
KIN Rehabilitation & Homecare launches Thailand’s first seamless Continuum of Care model for comprehensive stroke rehabilitation.
Thailand is becoming a fully aged society, while stroke remains a major cause of disability and loss of quality of life among patients of all ages. The challenge is not limited to hospital treatment, but includes continuous rehabilitation after hospital discharge where a major gap still exists in Thailand’s healthcare system.
KIN Rehabilitation & Homecare designed the Continuum of Care model to close this gap by collaborating with advanced neuro-rehabilitation technology partners and creating a seamless stroke-care pathway from the first day of rehabilitation until the patient can return to everyday life.
1. What Is the Continuum of Care Model?
Brief Answer
KIN Rehabilitation & Homecare developed the Continuum of Care model to provide stroke patients with comprehensive, continuous care by connecting 3 main systems along one pathway. The pilot programme is based at KIN Origin Physical Therapy Hospital, Bearing, and KIN Ladprao 71.
KIN Rehabilitation & Homecare was founded in 2018 by Dr Thongchai Chokthanomsap and currently operates 6 branches nationwide: Ladprao 71, Bearing (Sukhumvit 107), Ratchaphruek, Pattaya, Ramkhamhaeng 24, and Salaya, together with HomeCare services nationwide. KIN has cared for more than 6,000 families through a team covering 4 medical specialties and more than 10 multidisciplinary disciplines.
KIN Rehabilitation & Homecare provides Stroke Rehabilitation, Nursing Home, HomeCare, Hydrotherapy, Aquatic Treadmill, HBOT, and TMS services across 6 branches nationwide.
Systems Connected by KIN
Medical-Care Foundation
A nationwide network of rehabilitation centres, Nursing Home and HomeCare services, supported by more than 10 multidisciplinary disciplines and 24-hour nursing.
Rehabilitation Technology
TMS, Aquatic Treadmill, Hydrotherapy, HBOT, and advanced partner technologies for Neuro-Rehabilitation.
2. What Problem Does Continuum of Care Aim to Solve?
Brief Answer
Many stroke patients leave hospital and then experience a break in care because there is no single system connecting centre-based rehabilitation, specialist technology, and home care. As a result, they may miss the crucial 3–6 month “Golden Period” for recovery.
Medical evidence clearly confirms that the first 3–6 months after a stroke are when the brain has its highest neuroplasticity. Brain cells can develop new pathways to compensate for damaged areas, and patients who receive continuous rehabilitation during this period have a much greater chance of regaining walking, speech, and daily-life abilities.
Families of stroke patients in Thailand face these real challenges:
- The patient leaves hospital but still cannot walk, speaks unclearly, or has difficulty swallowing — where should the family go next?
- Different rehabilitation centres specialise in different areas, leaving families to search and coordinate care themselves.
- After returning home, there is no continuous-care system, and progress may decline again within only a few weeks.
- Costs continue without a clear endpoint because there is no defined treatment plan.
KIN designed a rehabilitation Patient Journey that combines the strengths of medical care and technology. Patients and families are supported under one Clinical Protocol from hospital discharge until the patient can return to life at home.
Advanced neuro-rehabilitation technologies made accessible to KIN patients through the Continuum of Care system.
Gap 1
Hospitals focus on acute treatment and are not designed for long-term rehabilitation.
Gap 2
Robotic technology and Neurotechnology are difficult to access, with no systematic referral pathway.
Gap 3
HomeCare is not sufficiently connected with medical teams and rehabilitation centres, causing interruptions in care.
3. How Does the 3-Step Continuum of Care Work?
Brief Answer
The model has 3 steps: (1) inpatient rehabilitation provided by KIN, (2) access to advanced technology through partner networks, and (3) continuous home care through KIN HomeCare. All 3 steps are seamlessly connected, so patients do not have to design their own treatment pathway.
Aquatic Treadmill at KIN Origin Physical Therapy Hospital, Bearing — KIN is Thailand’s first and only stroke rehabilitation centre to use an Aquatic Treadmill in patient rehabilitation.
Intensive Inpatient Rehabilitation — Care Provided by KIN
24-hour residential rehabilitation under physicians, nurses, and physical therapists, with an individualised stroke rehabilitation programme together with Hydrotherapy and Aquatic Treadmill and a full range of rehabilitation equipment at both KIN Origin Physical Therapy Hospital, Bearing, and KIN Ladprao 71.
Advanced Neuro-Rehabilitation Technology — Technology Partner Network
KIN refers suitable patients to access advanced neuro-rehabilitation technologies through partner networks, including Robotic Exoskeleton, upper-limb rehabilitation robots, VR, TPS, and brainwave analysis, all under the same Clinical Protocol.

Advanced brain and nervous-system stimulation technologies accessible to KIN patients through the partner network.
Long-term Home Rehabilitation — Care Provided by KIN HomeCare
KIN HomeCare provides continuous care at home nationwide — physical therapists, caregivers and nurses continue follow-up after discharge from the centre.
4. KIN’s Vision Behind Continuum of Care
Brief Answer
KIN believes the best outcomes do not come from a single device, but from a system that continuously connects every step—from physicians and technology to care at home.
“I believe Thai families are not looking merely for a ‘rehabilitation service’ — they are looking for ‘a chance for their loved one to return to life’. Until now, this pathway has been fragmented. KIN decided that Thai families should no longer have to navigate it alone, and that we would become the first in the country to create one genuine pathway from the patient’s bedside to the front door at home.”
Dr Thongchai Chokthanomsap — Chief Executive Officer and Founder, KIN Rehabilitation & Homecare

The pilot programme begins at KIN Origin Physical Therapy Hospital, Bearing (Sukhumvit 107) and KIN Ladprao 71 Both branches have physicians covering 4 specialties, 24-hour registered nursing, and a complete range of rehabilitation technologies. Call 02-096-4996.
How Is Continuum of Care Different from General Rehabilitation?
General rehabilitation often ends when the patient leaves a centre or hospital. Continuum of Care connects 3 continuous systems: Inpatient care → advanced technology → HomeCare, avoiding dangerous interruptions in recovery, particularly during the first 3–6 month Golden Period.
Can KIN HomeCare Provide Continuous Care Nationwide?
KIN HomeCare provides home-based patient and older-adult care nationwide, including physical therapy at home older-adult caregivers and private-duty nursing. Call 061-881-9399.
